For young people with chronic kidney problems who need to move on from children's services, Dr Ehidiamhen and Dr Simon Curran, Consultant Nephrologist at the Sheffield Kidney Institute (Northern General Hospital) provide transition support.  

There is a specific transition clinic that runs approximately every 6 months at the Northern General Hospital.
